Swan Hill District Health - Sub-acute care services

Swan Hill District Health’s expanded sub-acute care facility opened in May 2019.

The Victorian Government invested $2.6 million in the expansion. The project's aim was to improve patient access to the rehabilitation and care needed when recovering from a hospital stay, and live as independently as possible.

Funded via the Regional Health Infrastructure Fund, the expansion delivered an extra 14 beds. This enabled Swan Hill District Health to provide general inpatient rehabilitation services, geriatric evaluation and management services, alongside transitional and palliative care beds.

The Victorian Government’s $490 million Regional Health Infrastructure Fund is the largest of its kind in Victorian history. It is rebuilding regional and rural hospitals to ensure Victorians can access the high-quality care and facilities they need, no matter where they live.

The new sub-acute facilities development delivered 14 new beds. This enabled Swan Hill District Health to provide:

  • general inpatient rehabilitation services
  • geriatric evaluation and management services (multidisciplinary management of elderly patients with complex care needs)
  • transitional care beds (to support patients as they move from hospital to home or other facility)
  • palliative (end-of-life) care beds

 

The Swan Hill District Health - sub-acute care services project aimed to provide the following benefits:

Improved rehabilitation

The new inpatient services play a critical role in providing rehabilitation therapies that support greater independence. They aim to enable patients the ability to continue living at home.

Better management of illness

People with complex medical needs are supported to better manage their illness or injury at home, preventing future hospital re-admissions.

Compassionate end-of-life care

The new palliative care beds allow the Swan Hill facility to provide compassionate end-of-life care for those with terminal illness and support for their families, closer to home.

Improved pre and post-operative care

New perioperative equipment means patients can enjoy better health outcomes. It will also mean the hospital can continue to meet increasing demand for local endoscopies.

This project was delivered by the Victorian Health and Human Services Building Authority in partnership with:

  • DWP Suters (architect)
